What is wrong
Look at the newsletter box in your footer. If the only clue about what to type is the faint grey word inside the box, that word disappears the moment someone starts typing, and it was never readable by a screen reader in the first place.
The same usually applies to the contact form and to the order notes field at checkout.
What it costs you
A field with no label is a field people get wrong. They put the wrong thing in it, or they abandon the form, and you never find out because an abandoned form leaves no trace.
For someone using a screen reader it is worse than a guess. The field is announced as "edit text" with nothing else, so there is nothing to guess from.

We keep your layout. If the design depends on the label sitting inside the box, we keep that look and make it work anyway.
